  • Care Planning & Record Keeping Course

Care Planning is a non-negotiable clinical skill (NMC 2009) that all registered nurses must be competent at. Carers in residential homes who are expected to write care plans must also be competent at this skill. Everyone who documents in a patient/resident’s care records is accountable in law. Registered nurses are accountable to the NMC as well as to the law.

Documentation is the evidence used to demonstrate the standards and types of care being delivered. If documentation is not up to scratch or if aspects are omitted, then clinicians/organisations can be held to account as already mentioned, both legally and professionally. When the Care Quality Commission (CQC) carry out their regular inspections of care and residential homes they will justifiably come down hard on homes where the standards of documentation is poor. Many homes will achieve poor or adequate star ratings, and many currently face an embargo or even closure as a result of poor documentation.

Poor care planning and record keeping means that organisations are unable to defend themselves in cases of litigation, as it is this documentation that is examined in court. In many cases of litigation, the Claimant has/will win compensation amounting to thousands of pounds because documentation was/is not up to acceptable standards.Nurses are frequently struck off the NMC register for poor record keeping and carers expected to write care plans are increasingly held to account for their record keeping, resulting in litigious action against them and/or their employers.   GNCS will teach you and your staff on the art and clinical skill of care planning and record keeping in order to help prevent litigation against them and your organisation. The course teaches the systematic approach of assessment, re-assessment, care planning and evaluation. It looks at the activities of daily living and the use of commonly used assessment tools.

Your risk assessment of First Aid needs may well identify the need for Emergency First Aiders either as an addition to your First Aiders, or where the risk of your business is low, it may well identify them as being sufficient without the need of additional First Aiders.

  • Pressure Ulcer Prevention Course

Pressure damage in most cases can be avoided, yet the incidence of pressure damage remains high in all health care settings. This therefore leaves health professionals and organisations at risk of litigation and/ or loss of registration. Litigation in this area is increasing each year. The costs of treating pressure ulcers are high in terms of nursing time; hospital admissions with high financial costs in terms of wound care products, treatments and antibiotics.

This course will give delegates an awareness of those patients/clients at risk of pressure damage and will provide them with national and European guidance on preventative measures to minimise the incidence rates. This in turn will reduce costs and the risk of litigation as the incidence of pressure ulcers will be reduced with this knowledge.

  • Medical Law for Nursing Homes Course

This is of high importance to all health care providers, particularly with regard to the recently implemented Mental Capacity Act that come into force in April 2007. This Act brings into question issues surrounding consent, particularly with regard to the person without capacity; who can consent on their behalf?   Health providers must act within the law on issues such as photography, provision of nursing care, and confidentiality. This course will put all of these issues into perspective and is designed to provide the health worker/organisation with an awareness of their responsibilities with regard to the legal aspects of the care that they provide. For example, if a patient develops pressure damage, staff must ensure that they have taken all precautions to prevent this damage in the first place. Failure to do this could result in a medical negligence claim against the individual or the organisation. The course will also ensure that organisation managers/leaders are aware that accepting a patient into the organisation means that the patient is owed a duty of care by the organisation. By accepting the patient the organisation is stating that they can provide for that patients health care needs. Should an organisation fail in this duty of care that results in harm to the patient then this could result in prosecution of the organisation or individual.

  • Ask the Expert

Tissue Viability is about maintaining skin and underlying tissue integrity. In other words; keeping skin healthy and intact. Most people think of tissue viability as getting wounds to heal rather than how they develop in the first place.

Because of the high incidence of pressure ulcers occurring across all health care settings the role of a Tissue Viability Nurse (TVN) came into being many years ago in order to prevent such wounds occurring in the first place. Despite this, there remains a high incidence of pressure ulcers occurring needlessly. Many experts believe that 95% of all pressure ulcers can be avoided provided that nursing staff and/ or carers implement a programme of care to prevent such wounds.

Please visit www.gncs.co.uk and one of our experts will answer any question you may have.
